The first step of filing requires us to assess your claim. Please download and complete the following form, so that we may send you the proper program forms for your disability needs.
Apply By Mail
You can also apply by mail, where we initially send you the G1/G2 packet + the L1, which contains all of the information you need for a complete filing.
Apply By Office
You can always apply by coming into one of our office locations, and completing the necessary forms. Please review the offices, and choose and appropriate location based upon where you live.
